    Job Description:

    The IRCHSC G.R.O.W. (Guidance, Resource, Openhearted, Wisdom) Doula Program Assistant is responsible for assisting the G.R.O.W. Doula Coordinator and G.R.O.W. Doula Director in the local Indian River County Community Doula program and statewide implementation of the G.R.O.W. Doula Model ensuring high-quality, evidence-based practices, supervision, Doula care, case management, and maternal and child health programming.  

    Responsibilities include but are not limited to the following:

    -Assist with recruitment, screening, hiring, training, and supervision of G.R.O.W Doula subcontracted Doulas in Indian River County.

    -Identify training needs and opportunities; coach staff in working with healthcare providers, social service, and partners.

    -Backup to the G.R.O.W. Doula Coordinator if on call Doula cannot be reached.

    -Create and maintain the on-call Doula schedule.

    -Assist and support the G.R.O.W. Doula program and model in Indian River County.

    -Support the G.R.O.W. Doula Coordinator with monitoring, tracking and the completion of credentialing for all Doulas subcontracted with Indian River County Healthy Start Coalition to ensure Medicaid provider eligibility.

    -Monitor and track compliance for all Doulas subcontracted with Indian River County Heathy Start as assigned by G.R.O.W. Doula Coordinator.

    -Report to G.R.O.W. Doula Coordinator any subcontracted Doulas who are no longer eligible to provide services in Indian River County.

    -Act as a Backup Doula when necessary to Doulas subcontracted with Indian River County Heathy Start Coalition.

    -Contact potential Doula client referrals and assign to active subcontracted Doulas.

    -Support the G.R.O.W. Doula Coordinator in the assessment of program strengths and weaknesses and create plans for improvement.

    -Assist the G.R.O.W. Doula Coordinator in the completion of monthly Quality Assurance/Quality Improvement reporting on local G.R.O.W. Doula program to G.R.O.W. Doula Director and Data & Sustainability Coordinator in the timeframes specified.

    -Assist the G.R.O.W. Doula Coordinator in the monitoring and tracking of outcomes for grants and contracts, creating improvement plans when necessary.

    -Review Quality Assurance/Quality Improvement Service Delivery Plan to ensure program accuracy and compliance.

    -Compliance with the current model of care and ensure fidelity of the G.R.O.W. Doula Model in the local delivery of services.

    -Support the G.R.O.W. Doula Director with statewide implementation as needed.

    -Other Duties as assigned.

    Qualifications, Training and Experience:

    -Experienced Certified Doula

    -Intermediate proficiency in Microsoft Office.

    -Excellent verbal and written communication skills along with interpersonal skills.

    -Healthy Start Coalition Structure and system of care.

    -Maternal Child Health.

    Ability To:

    -Lead and model professional behavior to staff.

    -Work independently and meet deadlines accordingly.

    -Work well within a team environment

    Job Description:

    The Maternity Navigator provides a warm and welcoming entrance for pregnant and postpartum mothers and families. This position provides a single point of entry for access and referral of families into prenatal and postpartum services.

    This position is embedded in the prenatal care office and the hospital. Clients are connected to Maternity Navigation as part of their care with these facilities, provided referral, the Healthy Start Statewide Well Family system, community referrals or prenatal and infant risk screening tool.

    The position will serve a critical role in connecting with pregnant and postpartum mothers and families to supports needed to be the parents they want to be.

    Documentation, data collection and organization are a large part of this role.

    Also Responsible:

    -Assist in building self-assurance by answering question of pregnant and postpartum mothers and families either through evidence based trained knowledge or connecting to provider or resource.

    -Assist women to receive prenatal services by entering in and processing Presumptive Eligibility for Pregnant women (PEPW)

    -Accurately conduct the intake process with families for initial identification of service referral

    -Refer families to the appropriate home-visiting service provider

    -Follow up to ensure referral is received by service provider and services are provided to families

    -Provide immediate referral to community resources for families with emergent needs

    -Refer families to other community and parenting services as indicated

    -Follow up with all families to ensure questions are answered and connections have been made.

    -Maintain regular contact with services throughout the community.

    -Maintain participant records in the Well Family System in accordance with program and funding source requirements

    -Some drop by of home, shelters and other spaces outside office maybe necessary

    -Connect with other service providers to ensure correct documentation

    -Participate in continuous quality improvement activities

    -Maintain confidentiality of children, families and other records and information

    -Participate in trainings, meetings, workshops, conferences, webinars, and conference calls as necessary

    -Track outcomes and work with senior staff on QA/QI

    -When appropriate provide Healthy Families Assessment

    -Other duties as assigned

    You Have:

    -Mastery of Spanish and English language

    -Advanced degree preferred plus experience in the maternal health field, however a candidate with enhanced knowledge and experience will be considered.

    -Knowledge of child development and maternal health

    -Knowledge of eligibility criteria for home visiting programs in Indian River County

    -Experience with Healthy Families or Healthy Start Intake and assessments

    -Ability to connect with and interact effectively with culturally diverse families

    -Ability to work with families in a non-judgmental and respectful manner

    -Ability to communicate effectively with multiple community partners and medical providers

    -Ability to use good judgment and make independent decisions with clear and professional boundaries

    -Ability to effectively organize and prioritize work activities and complete tasks in a timely manner
