This Notice of Privacy Practices (the “Notice”) provides information on how your health information is used and disclosed by our programs and your rights regarding that information. Our team is committed to safeguarding your privacy and ensuring that your health information is kept confidential. This notice applies to the prenatal and postpartum care services we provide and is in<\/em> accordance with the Health Insurance Portability and Accountability Act (HIPAA), Florida State Law and regulations from the Agency for Health Care Administration (AHCA) and other applicable federal statutes.

Who We Are:<\/strong>
Indian River County Healthy Start Coalition offers prenatal and postpartum care services, but we do not perform deliveries. Our team and contracted providers travel throughout Indian River County, Florida to provide these services.

Your Health Information:<\/strong>
We collect and maintain your personal health information to provide you with prenatal and postpartum care services. This includes but is not limited to:

\u2022 Personal identification information (name, address, date of birth, etc.)
\u2022 Health history and medical records
\u2022 Prenatal and postpartum care details
\u2022 Test results
\u2022 Prescription information

How We Use and Disclose Your Health Information:<\/strong>
Your health information may be used and disclosed for the following purposes:

1. Treatment:<\/strong>\u00a0We may share your health information with other providers involved in your care, including specialists, community partners, contracted providers, laboratories, and the hospital where you choose to deliver.

2. Payment:<\/strong>\u00a0We may use and disclose your information for billing purposes, including coordination with your insurance company, Medicaid or other payers to obtain payment for services rendered.

3. Healthcare Operations:<\/strong>\u00a0We may use and disclose your information for our operational purposes, such as quality improvement activities, training, and audits to ensure that we continue providing you with the best care possible.

As a client, you have the following rights concerning your health information:

2. Right to Request Corrections:<\/strong>\u00a0If you believe your health information is inaccurate or incomplete, you may request that it be amended.

3. Right to Request Confidential Communication:<\/strong>\u00a0You can request that we communicate with you in a specific way or at a specific location (e.g., by phone, email, or in writing).

4. Right to Request Restrictions:<\/strong>\u00a0You may request that we limit how your health information is used or disclosed. While we are not required to agree to all requests, we will consider each request on a case-by-case basis.

Indian River County Healthy Start Coalition \u2014 Text Messaging Program<\/strong><\/strong>
By opting in to receive text messages from Indian River County Healthy Start Coalition (“we,” “us,” “our”), you agree to the following terms:

1. Program Description<\/strong><\/strong>
By providing your phone number and opting in, you consent to receive SMS text messages from Indian River County Healthy Start Coalition, including updates on programs, events, fundraising campaigns, donation confirmations, and other communications related to our mission. Participants who opt in to the Healthy Start text messaging program may receive weekly pregnancy education messages tailored to their stage of pregnancy, program reminders, surveys, event reminders for events they have registered for, and other Healthy Start program notifications related to maternal and infant health.

2. Message Frequency<\/strong>
Message frequency may vary.

3. Message and Data Rates<\/strong>
Message and data rates may apply, depending on your mobile carrier and plan.

4. Opt-In<\/strong>
By texting, providing your number through our website or donation form and other various platforms, or otherwise affirmatively opting in, you consent to receive recurring automated text messages from us. Consent is not a condition of any purchase or donation.

5. Opt-Out<\/strong>
You can opt out at any time by texting STOP<\/strong>. After opting out, you will receive one final confirmation message.

7. Supported Carriers<\/strong>
Carriers are not liable for delayed or undelivered messages. Supported carriers may include AT&T, T-Mobile, Verizon, and others; coverage may vary.

We reserve the right to update these terms at any time. Continued participation in the program constitutes acceptance of any changes.
